What this is. A short working-style questionnaire. You check the entries that describe you and we read the pattern across your answers.
Why we are asking you. We are checking this questionnaire against a commercial instrument some of us have already taken, to find out whether it measures what we think it measures. Your answers calibrate the questionnaire itself. This is not part of any hiring, review, or promotion decision about you, and it does not go into your personnel file.
What we keep. Your answers, the date, and which version of this questionnaire you saw. It is not shared outside the small group doing this work.
How long we keep it. Two parts, for different lengths of time. The copy carrying your name and email is deleted as soon as your answers have been recorded under a code, and in any case within 12 months of your submitting them. What remains is a coded record — your answers and the scores read from them, filed under a short code instead of your name. We keep that for as long as we use this questionnaire, because it is the reference set the calibration rests on.
Coded is not anonymous. One person holds the list linking your code to you, so that your result from the other instrument can be set beside this one, and so that we can still delete your record if you ask. We are not telling you that you cannot be identified — we are telling you your name is not in the file.
Removing it. Write to cheryl@synthweave.ai and we will delete it: the copy with your name on it and the coded record both.
Your name and email. We ask for them so we know whose answers these are, and so we can write back to you. They are part of the identified copy described above, deleted when it is and on the same terms, and nothing else about you is collected here.
